    Empowering Life Program Participant Agreement
    Class Expectations of Participant: The more you participate, the more you will get out of it! We learn from one another, so please treat others as you want to be treated. You matter and have important things to share. There is homework assigned every week. We have incentives to help you each week. To qualify, you need to be in attendance, participate in class and have your homework done.  If you need help, a mentor can be assigned to you if we have the volunteers to do so.
     
    Registration: If a non-Love INC client, I agree to pay the $20 registration fee which will include my class materials. If a Love INC client, I understand that I am not charged anything for this class.
     
    Timeliness: Classes begin promptly each night at 6:00 pm. Please have your materials ready. If you are more than 15 minutes late, you are asked to come at a different time to make up the class. We do not want to interrupt those who are already in session.
     
    Attendance: I agree to attend all the classes in my required class schedule (8, or 12 weeks). If I am not able to attend, I agree to contact the Ministry Coordinator by 12:30pm (320-391-5089) on the day of class. If I do not call ahead, I understand that my absence will be considered unexcused. For many classes, they are hands-on with group discussions and activities that are hard to recreate with just one or two people. A call, in advance would be appreciated so that we don’t worry!
     
    Homework: If I do not complete my homework and/or miss my mentorship appointments more than twice, I understand that I may be removed from the program.
     
    Frequent Absence: If I miss more than two classes of my required class schedule (8, or 12 weeks) as unexcused absences, I understand that I may be removed from the program. At that time, we will re-evaluate if this is the best time for you to take a class. We cover a lot of information each week and you won’t qualify for future incentives or graduation if too many classes are missed. Talk to the Ministry Coordinator if you have any questions or call the Love INC clearinghouse if you need further support! 320-759-3022.
     
    Referrals: I understand that if I have been referred to the classes by my church leadership/ or other agencies, and I am removed from the class, Love INC will notify my church/other agencies of the situation.
     
    Meals: We will offer meals before classes. The doors open at 5:00pm and dinner will be served at 5:15pm – 5:45pm. Classes start at 6:00pm.
     
    Confidentiality: Love INC takes confidentiality very seriously. I agree that I will not disclose or discuss information regarding other participants with anyone except Love INC personnel. If I have an issue with another participant I will speak with my mentor, class-instructor and/or Love INC staff.
     
    Bad Weather: If school District 206 is closed for severe weather, classes will be notified via text, email or via the Transformational Tuesdays Facebook page. If we miss more than one class because of the weather, we may have to schedule two classes in one night to make up for it. 
     
    Transportation: Love INC will provide transportation to clients as available by our volunteers when notified by the Friday prior to classes. If there is an emergency, such as a car accident or broken-down vehicle, please call or text the Ministry Coordinator’s cell, 320-391-5089.  We cannot guarantee a ride each week as they are based on availability of volunteers.
     
    Childcare: Childcare will be provided each night, pending volunteer availability. We need advance notice of which children will be attending so we can prepare. Please confirm each week the Monday before class which children will be present. We will communicate if there will NOT be childcare in advance to prepare participants. All volunteers that are serving in the childcare ministry will have background checks completed for safety.
